We help patients at the Capital District Psychiatric Center transition to community living by securing supportive housing, mental health treatment, primary care, disability benefits, and employment.
We provide support for young adults leaving the children’s mental health system by creating connections to adult mental health services such as housing, vocational training, primary care, as well as setting and accomplishing education and employment goals.
Annually CTT helps over thirty people living with severe and persistent mental illness gain independence and a higher quality of life through referral making, advocacy, teaching, and support.
